Student CARE Form

Reporting students of concern.

The Bellin College CARE Team provides faculty, staff and students a resource to contact if they have concerns about a student. The CARE Team will respond as quickly as possible to all referrals. The referrals will be monitored when college offices are open Monday – Friday from 8:00 a.m. to 4:30 p.m.  

This form is confidential and is only reviewed by members of the CARE Team. The more details you can provide in this form, the better care we can provide for the student. If the CARE Team needs additional information, we may contact you. Thank you for helping us support our students.
